What constitutes grave sin? How can I know if it is ok to receive the Eucharist?

Q: I was told that receiving the sacrament of the Eucharist while in a state of grave sin is a grave sin in itself. I’m constantly worried about whether I am allowed to receive Communion at Mass, and I do make sure I go to confession monthly (or more often). I am confused about the definition of contemplation, can you help me understand?

Q: Dear Sister Carmen, after reading some modern writers and Saint Teresa of Avila I am confused about contemplation. It seems like there is some confusion or lack of concern about definitions.
